چکیده

Although rare, epilepsy arising from the insula may produce ictal manifestations that can mimic temporal and extratemporal semiologies. If unrecognized, this might lead to some epilepsy surgery failures. Seizures originating in the insula (insular epilepsy) produce semiologies such as sensation of laryngeal/ chest discomfort, contralateral widespread unpleasant somatosensory auras, oro-alimentary movements, and dysphonic/dysarthric speech, which usually occur with fully preserved awareness. Insular involvement after ictal propagation of seizures arising from temporal or frontal areas may produce a similar semiology, or limited to the objective features as subjective sensations are often masked by dysfunction of consciousness and memory. Propagation of epileptic activity between mesial temporal structures and the ipsilateral insula is remarkably rapid, reflecting the dense connections between the insula and temporolimbic structures. Spread from the mesial temporal structures to the contralateral insula has beenmentioned in a study on technical details of insular implantation in SEEG. Here, we report a patient with ultimately bitemporal epilepsy. Using stereo-EEG, spread of ictal activity from the left mesial temporal region into the right insula and primary motor face area was demonstrated that resulted in mislateralizing semiological signs (vomiting and left face clonic movements).

Results from the intracarotid amytal procedure in patients with uni or bitemporal lobe epilepsy and mesial sclerosis.

This study discusses the results from the IAP and its lateralizing and prognostic value in patients with temporal lobe epilepsy (TLE) and mesial temporal sclerosis (MTS). Eighteen patients were submitted to the IAP. Fifteen had bitemporal and 3 unitemporal lobe epilepsies. All patients had MTS on MRI and in one there was an unrelated lesion in the contralateral hippocampus. Nine bitemporal pati...

The lateralizing value of IQ in mesiotemporal epilepsy: Differences between patients with unitemporal and bitemporal epileptiform discharges

PURPOSE We investigated the lateralizing ability of intelligence scores in mesial temporal lobe epilepsy (MTLE) patients according to the distribution of interictal epileptiform discharges (IEDs). METHODS This study enrolled 82 MTLE patients. All patients had preoperative neuropsychological evaluations, including Korean Wechsler Adult Intelligence Scale. Patients were categorized as having un...

Lateralizing value of interictal spikes on overnight sleep-EEG studies in temporal lobe epilepsy.

PURPOSE To determine the lateralizing value of interictal epileptiform discharges (IEDs) recorded during overnight sleep-EEG studies in temporal lobe epilepsy. Because IEDs are more prevalent in non-rapid eye movement (NREM) sleep than in wakefulness, overnight sleep-EEG recordings may contribute additional lateralizing information to the epilepsy surgery evaluation beyond daytime EEGs. METHO...

Analysis of ictal semiology: contralateral tonic activity associated with ipsilateral automotor seizure in temporal lobe epilepsy

Ictal semiology is essential for the pre-operative evaluation of candidates for epilepsy surgery. We propose an ictal sign of temporal lobe epilepsy (TLE) termed tonic-automotor (TA), defi ned as contralateral tonic spasm of upper limb associated with ipsilateral hand automotor seizure and often accompanied ipsilateral head turning. We assessed the occurrence and lateralizing/localizing value o...

Lateralizing value of unilateral relative ictal immobility in patients with refractory focal seizures – Looking beyond unilateral automatisms

PURPOSE Ictal motor phenomena play a crucial role in the localization of seizure focus in the management of refractory focal epilepsy. While the importance of unilateral automatisms is well established, little attention is paid to the contralateral relatively immobile limb. In cases where automatisms mimic clonic or dystonic movements and in the absence of previously well-established signs, uni...
